Our Family Health Team has partnered with the Ottawa Model for Smoking Cessation (OMSC) to help you in your quitting smoking journey. This program supports nicotine users to reduce or quit.

Whether you smoke tobacco or use an e-cigarette, we can help you reduce or quit your nicotine use. Smoking Cessation Counsellors provide individual, one-to-one counselling sessions. We encourage, assist, and educate patients in a supportive, non-judgmental environment. You can choose whether you prefer in-person, virtual or phone appointments. 

What to Expect from Our Program

Initial Quit Plan Visit: 45-minute session (in-person, virtual or phone)                  

  • This is an individual session with a smoking cessation counsellor where you will:
    • Explore the reasons why you smoke or vape 
    • Identify your triggers 
    • Learn strategies to help you avoid smoking or vaping
    • Discuss and learn how medications can help you, if this is right for you 
    • Receive a free 6-week supply of nicotine patches and lozenges or gum, if this is right for you 
    • Receive a personalized quit plan 

Follow-up Visits: 20 to 30-minute session (in-person, virtual or phone)

  • These are individual sessions where you will:
    • Receive ongoing support from a smoking cessation counsellor (up to 6 follow-up sessions) 
    • Create an ongoing plan to help you remain nicotine-free 

How do I register for this program?

You must be a patient of the TW FHT to register for this program. You can register online below or get referred by your healthcare provider. 

What other smoking cessation programs are there in the community?

Nicotine Dependence Clinic

The Nicotine Dependence Clinic offers an assessment, medical consultation, group counselling and medications to quit/reduce smoking. Their clinic provides service to clients with concurrent substance use and/or mental illness as well other medical conditions.
