The Toronto Western Family Health Team (TW FHT) was established in 2007 and is built on a long-standing family medicine department at the Toronto Western Hospital and University Health Network (UHN). We are partnered with the Department of Family and Community Medicine (DFCM), an academic teaching unit fully affiliated with the University of Toronto. The TW FHT is overseen by its Board of Directors, which is responsible for its overall governance.
TW FHT is an interprofessional team-based primary health care organization. This means that we have different health care professionals with unique skills and expertise that work together to give you the best possible care.

How We Improve Patient Care
We offer a wide range of programs and services including health promotion services, chronic disease management programs, a home-based care program, and an obstetrical care program.
We are highly engaged in the training of medical students, residents and other health professional trainees.
We are involved in research and quality improvement, as well as patient safety initiatives.
We have partnerships with several community organizations to plan and deliver services to a wide range of programs and population groups.
Who We Care For
Our team of health care professionals work across two clinical practice sites:
Bathurst Site located at 440 Bathurst Street, 3rd Floor. We serve the communities from Bloor Street to Lake Ontario; Dufferin Street to Yonge Street.
Garrison Creek Site located at 928 St. Clair Avenue West. We serve the communities from Eglinton Avenue West to Bloor Street; Keele Street to Spadina Avenue.
