Anyone coming in for testing must have a valid health card and a TeamUHN member accompanying them along with their UHN ID badge.
Please read the email sent after booking an appointment inFULL prior to your appointment as this contains crucial steps for performing the self-swabbing test and what is needed to perform this test.
Staff onsite will NOT be able to help perform this test on site.
Also,ONLY those who are TeamUHN members, orTeamUHNfamily members orTeamUHN household members can complete a self-swabbing test within our sites.
Once the test is complete, you must exit the building immediately. Individuals with symptoms consistent with COVID-19 are presumed positive and they should follow isolation and/or self-monitoring guidelines.

COVID Care for members of TeamUHN (Self-Referral available)
To ensure members of TeamUHN and their household have timely access to COVID care, the Connected Care Clinic is available to support assessment and follow-up care; this includes access to COVID-19 therapeutics for high-risk individuals and most visits are conveniently supported virtually. More information is available in the email confirmation that will be sent out after booking this appointment.
